A Study of Home Nursing Care Needs


Abstract
The purpose of this study is to provide the basic information for home nursing care practice and the system by identifying home nursing care needs, environmental recognitions, information needs and basic nursing problems related to the health problem of chronically ill patients.
Home health care is that component of comprehensive health care whereby services are provided to individuals and families in their places of residence for the purpose of promoting, maintaining, or restoring health, or minimizing the effect of illness and disability. There are many psychological benefits to the client who receives health care service at home.
The subjects for the study were selected from chronically ill patients in the medical outpatient department of a general hospital, a university hospital and five public health centers in Seoul March 22 through April 4, 1989. 80 clients under medical insurance coverage and 63 clients of medicare, and 18 clients of other medical expenditure answered the questionnaire.
The result can be summarized as follows.
1. General characteristics of clients.
The majority of subjects are female (69.6%).
35% of medical insurance clients are in their fifties, and 33.37o of medicare clients are in their sixties.
27.57o of medical insurance clients and 31.7% of medicare clients are elementary school graduates.
By occupation, 42.5 % of medical insurance clients are housewives, 44.4% of medicare clients are laborers; in terms of their monthly income, 23.8% of medical insurance clients earn 200-400 thousand won and 23.8% of them 400-600 thousand Won, and 82% of medicare clients ¢¥less than 200 thousand Won.
In the health problem, 31.1 To of subjects are cardiovascular disorder clients, 24.80Jo gastrointestinal disorder, 1437o respiratory disorder, and 11.87o endocrine function disorder
clients.
2. The recognition of home nursing care by public health nurse.
1) With public health nurse visiting their home, 75 07o of medical insurance clients¢¥ and 49.2 % of medicare clients wish to discuss their physical health.
2) In response to home nursing care by public health nurse, 72.3% of the clients-want home ; nursing care, 20.8% of the clients do not it.
3) 48,8% of the medical insurance clients and 68.3 To of medicare clients find necessity of home nursing care by public health nurse.
4) The expenditure of medical treatment lead to their refusal of home nursing care (36.3,0) j 33.3% says it is unnecessary.
3. The living environments and information needs related to the health problem.
1) As for the living environments closely related to health care, lots of medical insurance clients find satisfaction in their living condition (63.8 %) and most medicare clients complain of their kitchens (25.4Io ). -j
In the disturbance factors of family health, 28.807o of medical insurance clients indicate their dietary habit, 31.7% of medicare clients their health knowledge deficiency.
2) In emergency, most medical insurance clients prefer visiting general hospital (49.8%) and most medicare clients local clinic (34.9%).
In normal times, 42.57o of medical insurance clients would` visit pharmacy, 63.5% of medicare clients public health center.
3) As for information needs related to health problem, 33.5 To of the clients warit to know the exact and detailed cause of their disease and, 48.4% the efficiency of the prescribed medicine, 41.6 % the influence of food nutrition on their health, 34.2 % how to do treatment or exercise, 33.5 To how to prevent chronic illness Iike DM or hypertension.
4. Basic nursing problems.
1) Nursing problems indicate 24.9% in whole category, basic nursing problems indicate hi;h level in mental health and sensory problems (33.707o), circulatory and respiratory problems (27.4%), sleep and rest problems (27.1%).
2) Basic nursing problems related to health problem indicate high level in cardiovascular disorder, musculoskeletal disorder, disturbances of endocrine function.
